The short version

This website does not use advertising trackers, does not sell data, and does not set marketing cookies. Fonts and images are served from this site rather than a third-party network, so loading a page does not report your visit to anyone else.

The contact form is not a secure channel for medical information, and you should not use it for that. See the Notice of Privacy Practices for how protected health information is handled once you are a client.

Information you give us

If you complete the contact form, we receive the name, email address, phone number, preferred contact time, and message you enter. We ask you not to include symptoms, diagnoses, or medical history in it.

If you request an appointment, you are taken to a separate intake form operated by a third-party provider under agreement with the practice. Information you enter there is governed by that agreement and by the Notice of Privacy Practices.

Information collected automatically

The hosting provider keeps standard server logs, which may include your IP address, browser type, the pages requested, and the time of the request. These are used to operate and secure the site, not to build a profile of you.

Third parties

Appointment requests are handled by a third-party intake form provider. The practice maintains a business associate agreement where one is required.

This site links to external organisations such as crisis lines, state services, and a professional directory. Their privacy practices are their own.

Your choices

You can use the phone number instead of any form on this site. You can ask what information the practice holds about you and request a correction. Contact details are on the Contact page.

Review status

This document is a working draft prepared with the website. It must be reviewed and approved by the practice’s attorney or compliance advisor, and given an effective date, before the site goes live.
